Objective: To assess the safety and effectiveness of combined hormonal therapy in patients with occult recurring gastrointestinal bleeding of unknown origin. Methods: This a prospective observation research. It was done through a private system of extemal patients, affiliated members of an important hospital. Patients: 43. 14 Men and 29 Women. Average age: 74 years (ranging from 48 to 86). They have had prior episodes of occult gastrointestinal bleeding of unknown origin for over a year with multiple hospitalizations and transfusions. Initially, patients were given treatment with Enovid 5mg. which contains: 5mg. of norethynodrel and 75 micrograms of Mestranol. As Enovid was locally out of stock, treatment was conducted with Ortho-Novum 1150, one tablet twice a day, with contains: 1 miligram of norethinedrome and 0.5 miligrams of Mestranol. Patients were given treatment and observed for an average time of 535 days (between 25 and 1551 days). All Patients performed their own control and we made a continued periodic observation of their hematocrit, serial of fecal occult blood testing, report of medicine intake and clinic histories of transfusions and hospitalization due to bleeding or anemia. Results: Of 43 initial patients, 38 were treated with combined hormonal therapy. The other five were given treatment with Estrogen. In 25 patients, an initial Enteroscopy revelead AVMs in the stomach or small intestine and these were cauterized. No bleeding was found in the remaining 18 patients. None of the 38 patients treated with combined hormonal therapy bled while following the prescribed dosis . All five patients treated with estrogen had episodes of recurrent bleeding. With reference to the cauterization of the AVMs it was found that there was not any statistical difference in the recurrent bleeding level between those patients that were Conclusion: In this long-term observation research, it was shown that the combined horinonal therapy stopped the recurring bleeding in patients with occult Gastrointestinal bleeding of unknown origin |
