Leiomyoma is the most frequent extramuccosal intramural tumor of the esophagus, most of the cases is located in the third distal. Its incidence is variable and ranges from 0.005 to 5.1 % . It is more common in males by a ratio of 2:1 and a peak incidence in the third to fifth decade of life. In !0% of the cases they are multiple . Their sizes widely, from less than one cm in diameter to 29 cm . From 10 cm on they are considered giants . The treatment is surgical removal, either by minimally invasive techniques, endoscopic or thoracolaparatomy according to tumor size. We report a case of a young patient with a giant leiomyoma which was surgically resolved with resection of the tumor mass that involved the 2/3 of the lower esophagus and 1/3 of the proximal stomach by a thoracolaparotomy and esophagus gastric anastomosis. Keywords.- Giant esophageal leiomyoma
