Because of cancers high mortality around the world , the vailable therapeutics for neoplasic disease has increased, with a wide number of patients exposed to a great variety of anti-neoplasic drugs. Historically it has been known that anti-neoplasic agents could have adverse effects upon multiple organs and normal tissue. The most common associated toxicity is seen upon fast division cells which rapidly reverse the toxic effect with minimum long term toxicity. However myocardium cells have a limited regeneration capacity, which turns the heart a organ susceptible to present permanent or transitory side effects. This toxicity implies variable rates o f disorders, from benign asymptomatic arrhythmias, to conditions as lethal as ischemic lesions, infarctions y myocardiopathy. We shall review some cardiac toxicity syndromes associated with different anti-neoplasic agents. |

Introduction: Ultrasound (US) has become the most used imaging technique for evaluation of thyroid gland nodules, because the accessibility and great diagnostic value in the detection and differentiation of benign and malignant thyroid pathologies. The objective of this study was to determine findings of the US Duplex for the diagnosis of malignant pathology of the thyroid gland. Materials and Methods: A prospective, observational study was done for the diagnosis with US of malignant nodular thyroid disease in 100 patients selected from those in the ION SOLCA, Guayaquil, in the period within May, 2006 and October,2007. We correlated the findings of the US Duplex Doppler and US Power Doppler with the results of the cytology analysis. Results: From the total 100 cases in the study, 20 were positive for malignant disease, 71 were negative y 9 were suspicious. We found thet the following US characterics are predictive for malignant disease_ heterogeneity 20/20 (p<0.001, Sensitivity 100%, Specificity 60.5%). Irregular borders were present in 13 of 20 ,malignant nodules (p<0.001, Sensitivity 100%, Specificity 60.5%). Inrregular borders were present in 13 of 20 malignant nodules (p<0.001, Sensitivity 65%,Secificity 100%). Hypoecogenicity, 17/20 malignant nodules (p<0.001, Sensitivity 85%, Specificity 56.3%). Intranodular microcalcifications 13/20 ( p<0.001, Sensitivity 45%, Specificity 92.9%). The absence of halo sign 19/20 ( p<0.005, sensitivity 95%, Specificity 43.6%). With the US Duplex Doppler and US Power Doppler four vascular patterns were established: I, absence of vascular flow; II, presence of perinodular flow; III, presence of intra- and perinodular flow 13/20; and IV, only intranodular flow related to pattern III (13/20) AND IV (3/20); (P<0.001, sensitivity 85%, Specificity 66.2%). The presence of exclusive perinodular flow its related to benign disease (p<0.005, sensitivity 85%, Specificity 7%) Conclusions: Findings from US, Duplex Doppler and Power Doppler are highly predictive of malignancy when multiple singns are found simultaneosly inside a thyroid nodule. Introduction: Gastrointestinal tract is the most commonly affected extranodal non-Hodgkin lymphoma and about 60% of cases occur in the stomach. Primary gastric lymphoma ( LGP ) is defined as lymphoid neoplasia affecting stomach only and can involve the lymph nodes, but not mediatinal or retroperitoneum. Materials and Methods: A review of 5 medical records of patients treated in the National Oncology Institute with Primary Gastric Lymphoma. Chemotherapy protocols, time course and response to standard treatment were analyzed, and so surgery as a complementary o definitive treatment in patients who did not respond to initial treatment. Result: Four of five patients had Helicobacter pylori positivity ( blood) and received treatment. CT scan of theree patients reported asymmetric wall thickening of stomach, the esophagus-gastroscopy revealed various gastric mucosal lesions (localized or diffuse). Pathology reported three patients with HIGH GRADE NON-HODGKIN GASTRIC LYMPHOMA and Two as DIFFUSE NON-HODGKIN LYMPHOMA B CELL, MALT TYPE. Three received with 6 cycles of neoadyuvant chemotherapy with a duration between 6 to 8 months, after which endoscopic controls revealed persistent disease at pathology study, reason why surgical treatment was indicated. Two patients did not receive neoadjuvant treatment and were sent directly to surgery, one for active gastrointestinal bleeding tumor, and another 80-years old cardio patient who did not accept neoadyuvant treatment. The surgical treatment was total radical D-2 gastrectomy plus esophagus yeyuni anastomosis in Y Roux. The follow up shows that the five patients are still alive with a good oncologyc evolution, in periodic controls after four years the first one and eight months the last one after surgery. Only 1 patient developed anastomotic stenosis 1 month after surgery that resolved with endoscopic dilatations. Conclutions: It is a need to establish, identify and ratify the protocols of the ION (SOLCA) for Primary gastric lymphoms treatment, reminding the importance of surgical treatment as alternative complementary o definitive treatment for patients who not respond to traditional chemo or radiotherapy. Key Words.- Primary gastric lymphoma, Helicobacter pylori, gastrectomy, esophagus-gastroscopy, neoadyuvant.  |

Introduction: Breast cancer in Cuba ocuppies first cause of death amoung females. This location has been poorly studied in third age. Materials and methods: A descriptive retrospective study was dine about the clinical characteristics, therapeutical conduct and results in 70 years old patients with breast cancer and treated at the INOR during the period of 1992-2002.Population was199 pacients that achieved inclusion criteria. Frequency was calculated as so as qualitative variable porcentages, media and standard deviation of quantitative variables. Results: The mayor frequency was in 70-74 years, stage II, ductal invasive carcinoma. Most used therapeutical election was combined surgery + hormones, the type was conservative surgery. In the treatment election age had a higher significance than staging. Survival was significant according to: surgery + hormone treatment, stage 1, and age 70-74, as so as hormone treatment in elders than 80. Axilar dissection and radiotherapy after consevative surgery turned out to be pronostic factors in both events. Conclusions: There was a better survival rate within the group to which we applied combined surgical therapy with hormones, by which we consider it the ideal treatment for this group. Key words: Breast cancer, age beyond 70 years. |

Introduction: Invasive ductal carcinoma is the most frequent malignant tumor of the breast and its behavior is heterogeneous. Different therapeutic groups have been established depending on the morphologic features. Immunohistochemistry has opned new therapeutic optiions but the results are not contasted enough. Material and methds: We have correlated tumor size, combined histological grade, axilar ganglionar status and p53, MIB1, HER2/neu and expression of estrogens receptors in 107 patients with invasive ductal carcionoma of the breast. Results: There is statistic correlation between expression of estrogens receptors and nuclear grade, mitotic index, tubular differentation, histological grade, axiliar ganglion status and metastatic lymphatic ganglion group. There is not correlation with the tumor size. There are not significant alterations in p53 expression when it is correlated with the different morphologic parameters, only with tubular differentiation. El MIB1 expession correlates with nuclear grades, mitotic index. HER2/neu (+), especially in the group>4. Conclusions: The tumors receptors (+) have statitic carrelation with histological high grade and axilar ganglion status (+). The Tumors Her/2neu (+) have statistic correlation with histological high grade and axilar ganglion status (+), especially in the group >4. Abstract: | |
Hemangiolymphangiomas are relatively uncommon congenital deformities of both lymphatic and blood vessels. These bening tumors usually presentthemselves in the first years of childhood; most of them appear in the head and neck area. They originate fro an abnormal embriologic development of the lymphatic and vascular system. Its treatment may be diverse, but the most frequent and effective one is the surgical approach. Other options are laser evaporation or sclerotherapy, but they are still symptomatic and in some cases palliative treatments. Mortality varies from 0 to 6 % and it`s almost exclusively in cases of cavernous lymphangiomas in newborn babies. We report a rare case of a 44 year old patient with a clinical history of several years of evolution that produces left upper quadrant pain and anemia. Several Hematological, radiological and anatomopathologica, radilogical and anatomopathological exams suggest the presence of an intraabdominal as Systemic Hemangiolymphangiomatosis Key words: Hemangiolymphangioma, ascular malformation, lymphatic malformation.  |
