En esta única edición del volumen 20 de la revista Oncología, que abarca los 4 números del año 2010, nos ponemos al día con las ediciones periódicas que el Órgano oficial de comunicación científica de los distintos centros que constituyen la Sociedad de Lucha Contra el Cáncer (SOLCA) del Ecuador venía manteniendo y que se vieron interrumpidas por la larga enfermedad y luego prematura desaparición de su Editor fundador, Dr. Jaime Sánchez Sabando. |

El desafío de una ancianidad saludable ha dejado de ser una excepción y privilegio para convertirse en una oportunidad en muchos países. La declinación gradual de las condiciones de salud física y mental que acompañan al proceso de envejecimiento individual, la reducción resultante de la esperanza de vida activa y saludable, la reducción o cesación completa de participación en la vida laboral y la mayor dependencia de transferencias de ingresos de fuentes públicas y privadas, determinan que el crecimiento de la población más vieja debe conducir a una creciente demanda de servicios de atención de salud. Dado que la mayoría de las condiciones de salud de las personas mayores son crónicas en vez de agudas, y progresivas en lugar de regresivas, esta demanda también podría implicar un marcado aumento de los costos de atención de la salud. The challenge of a healthy old age is no longer an exception nor a privilege but has become an opportunity in many countries. The gradual decline of the physical and mental health conditions that goes with the individual’s aging process, the resulting loss of an active and healthy life expectancy, the reduction or complete cessation of participation in working life and the greater reliance on income transfers from public and private sources, determine that the growth of the older population should lead to an increasing demand for health care services. Since most health conditions of the elderly are chronic rather than acute and progressive rather than regressive, this demand could also cause an important increase in the costs of the health care.

Introducción.- El Registro de Tumores de SOLCA Guayaquil es considerado como la fuente de epidemiología descriptiva del cáncer, por dar a conocer y monitorizar la tendencia de esta terrible enfermedad. Introduction.- The Tumor Registry of SOLCA - Guayaquil is considered the source of the descriptive epidemiology of cancer, to raise awareness and monitor the trend of this terrible disease. The study aimed to determine the incidence of cancer in the city of Guayaquil.
Material and Methods.- Retrospective and descriptive study, wich collected new cancer cases in residents of the city of Guayaquil from 2003 to 2006. Data analysis and calculation of incidence rates were done by the Hospital Management System, owned by SOLCA.
Results.- In Guayaquil, from 2003 to 2006 there were 9647 new cases of cancer, 40.4% men and 59.6% women. Cervical cancer ranks first in women with 28%, in average 400 new cases were diagnosed per year, followed by breast cancer with 17% Prosstate cancer ranks first in men with 20.1 % skin cancer ranks second with 13.9% and in women ranks third 9.0. Stomach cancer in men is third with 12.2% and in women fourth with 6%. Lung cancer mainly affects men, a ratio 1.6/ 1 . Thyroid cancer is increasing yearly, on average 188 new cancer cases are diagnosed cancer i women and 10 men. Leukemias, rank first among children aged 0 to 14 years old, 46.8% and 44.5% respectively.
Conslusions.- Cases of cancer increased by 27 .6% in Guayaquil. The most frequent cancer in women is the cervix while in men is prostate cancer and leukemia in children. On average each year 1438 new cases of cancer in women and 974 in men are diagnosed. The male / female cancer is 0.7 / 1.

Introducción.- El tumor sólido pseudopapilar del páncreas es una neoplasia incierta, de etiología desconocida y de baja frecuencia, con predominio en el sexo femenino, de escasa sintomatología y buen pronóstico tras la cirugía radical. El objetivo del presente estudio es reportar la experiencia en 8 casos de esta patología de muy rara frecuencia y comparar los hallazgos de nuestra serie con lo reportado por la literatura consultada. Introduction .- Solid pseudopapillary tumor of the pancreas is an uncertain neoplasm of unknown etiology, of low-frequency, predominantly female sex, poor symptomatology and good prognosis after radical surgery. The aim of this study is to report the experience in 8 cases of this rare pathology and compare the findings of our study with that reported in the literature Material and methods.- We reviewed 8 cases of patients over 12 years with a diagnosis of solid-cystic papillary tumor of the pancreas surgically treated at the Department of Surgical Oncology National Cancer Institute SOLCA Guayaquil. The monitoring
data and long-term survival were obtained by reviewing medical records and outpatient control of surgical patients. We used central tendency measures for the statistical analysis.
Results.- The mean age was 30 years predominantly female. The symptomatology was nonspecific, but the most reported symptom was diffuse abdominal pain. CT and MRI revealed cystic pancreatic solid mass with predominance in tail (3 / 8). In 50% of the patients pancreaticoduodenectomy was performed resulting in a 0% of mortality.
Conclusions.- this is a very rare pathology, more common in women and unknown etilogy. It is caracterized by well delimited tumours with calcifications and hemorrages areas. The diagnosis is due to its histology. The surgical removement is normally curative.

Introduction.- Malnutrition is common in cancer patients, therefore nutritional support during tratment produces a favorable response and reduces complications.
The objective of this research was to determine whether oral supplementation administered in fractions to a first group of 30 patients at risk or malnourished, could improve nutritional status, compared with a second group of 30 patients who received supplementation administered completely.
Material and Methods.- Descriptive study i which we determined the nutritional status of outpatients at the Clinical Nutrition Service - Guayaquil SOLCA fron february to July 2010 oral supplementing them with polymerec formulas. For data evaluation software was used Excel and Chi square test.
Results.- We evaluated 60 cancer patients, whose average age was 65.7 years old, accounted for 51% male and 49% female. Stomach cancer was more frequent in 25% of cases. 43.3 % of all patients for risk of malnutrition, 21.6 % second - degree malnutrition, 21.6% second - degree malnutrition.
the administration of the supplement in installments improved their nutritional status, 23 (76.7%) of patients in the first group versus 12 (40%), the second group, with associated p value of 0.002 was satistically significant.
Conclusion.- Positive impact on patients in the group receiving oral supplementation in fractions improving their nutritional status. In addition they showed a better tolerance to the formula given.

Introducción.- El cáncer de hígado constituye una grave afección de dicho órgano. Su frecuencia de aparición es muy elevada en lugares como África Occidental y Asia. También se presenta en Francia y Estados Unidos aunque con evidencia menor. En Cuba, en el año 2008, ocupaba el 8vo lugar, en incidencia junto al de vías biliares, entre todos los cáncer de nuestro país. Se persigue como objetivo conocer la incidencia de esta patología, según edad, sexo y raza, los síntomas más frecuentes, complicaciones, pronóstico, los diagnósticos complementarios más utilizados, así como establecer el índice de mortalidad en el periodo analizado. Material y Métodos.- Se revisaron 65 historias clínicas de pacientes portadores de carcinoma hepatocelular, atendidos en el Servicio de Cirugía, en el Instituto Nacional de Oncología, en el periodo 1998 - 2003. Resultados.- Hubo predominio del sexo masculino, la mayor parte de la casuística estaba comprendida entre los 50 y 80 años, la raza predominante fue la blanca, y el síntoma más frecuente, fue la hepatomegalia dolorosa. Conclusiones.- La limitada efectividad de los recursos terapéuticos actuales contra el carcinoma hepatocelular y su diagnóstico tardío, hace que las tasas de sobrevida global, para estos pacientes sean muy bajas.

Introducción .- Liver cancer is a serious condition that organ. Its incidence is very high in places like West Africa and Asia. It also occurs in France and the United States but with less evidence. In Cuba, in 2008, occupied the 8th place, with the incidence of bile duct cancer among all of our country. It pursues the objective determine the incidence of this disease, by age, sex and race, the most common symptoms, complications, prognosis, diagnosis most commonly used complementary and establish the mortality rate in the period analyzed. Material and Methods.- We reviewed medical records of 65 patients with hepatocellular carcinoma trated at the Surgery department at the National Cancer Institute in the period 1998 to 2003. Results.- There was a predominance of males, most of casuistry was between 50 and 80, the predominant race was white, and the most frequent symtom was painful hepatomegaly. Conclusions.- The limited effectiveness of current therapeutic resources against hepatocellular carcinoma and its late diagnosis, causes the overall survival rates for these patients are very low. Introducción.- Al aumentar la expectativa de vida de la población, y disminuir la mortalidad por enfermedades infectocontagiosas, el cáncer se sitúa entre las principales causas de muerte en los países desarrollados, y Cuba no escapa a esa propensión. Por lo que nos propusimos caracterizar la mortalidad por los tumores malignos más frecuentes en el adulto mayor del municipio Plaza de la Revolución durante el quinquenio 2004-2008. Introduction.- With increasing life expectancy of the population and reducing mortality from infectious diseases, cancer is among the leading causes of death in developed countries, and Cuba is no exception to this tendency . Therefore we set out to characterize the mortality from most common malignant tumors in the elderly of the municipality of Plaza de la Revolution during the five years between 2004 and 2008.
Material anfd Methods.- We performed a descriptive study to characterize the behavior of mortality from most common malignant tumors in
the elderly of the Plaza de la Revolution during the five years 2004 2008. Variables were considered: anatomical location, sex, age and health area. The data processing was performed using the database created using SPSS 16.0 for Microsoft Windows Statiscal measures used were percentages, ratios, proportions and rates.

Results.- The locations that occupied the top cause of death in men were lung, 294.8 per 100.000 habitants and prostate, 254.1 per 100.000 habitants, in women 116.5 per 100.000 habitants for lung and breast 112.1 per 100.000 habitants. Depending on age, 75 and over, was the most affected in both sexes.
Conclusions.- The anatomical sites of malignant tumors that caused more deaths in adults over the Plaza de la Revolutions in the period 2004-2008 were lung and prostate in men and lung and berast cancer in women. In both sexes, with prevelance over 75 years.

Introducción.- El cáncer de Mama en mujeres menores de 35 años, presenta un reto particular, pues son tumores biológicamente más agresivos y se asocia con factores pronósticos desfavorables comparado con la enfermedad en pacientes de mayor edad. La mayoría son tumores poco diferenciados (alto grado), alta fracción de proliferación y mayor invasión vascular. Palabras Claves: Cáncer de mama, menores de 35 años, tratamiento, sobrevida.

Introduccion.- Breast cancer in women under 35 years becomes a particular challenge because they are biologically more aggressive tumors and are associated with more adverse prognostic factors compared to the disease in older patients. Most tumors are poorly differentiated (higher risk), high proportion of vascular invasion and higherproliferation Material and Methods.- We conducted a descriptive study aimed to characterize this group of patients, women younger than 35 years old were studied, and they were enrolled in the INOR diagnosed with breast cancer in the period 1996 - 2000. Prognostic factors were analyzed and related to relapse. Results.- Most patients were nulliparous (56.1%) 40.4% of patients relapse, nodal status and tumor size influenced relapse, infiltring ductal carcinoma was the most frequenty variety. The overall suvival was 84.6 and 73.1 at 3 and 5 years respectively. Conclusions.- Breast cancer in young women has incresed in recent years. Patients wiht larger tumors and positive lymph nodes had more relapse infiltrating ductal carcinoma was the most frequent histological subtype, although the mucinouns carcinoma had the highest number of relapses.

En esta última década los avances y la utilidad de la laparoscopía han sido muy satisfactorios para los cirujanos, sin embargo la pancreatoduodenectomía o Técnica de Whipple, era considerada como un procedimiento fuera de las posibilidades de la laparoscopía por la mayoría de los cirujanos. El presente estudio descriptivo, es llevado desde el mes de Agosto a Noviembre del 2010, operándose 2 pacientes por abordaje Laparoscópico, el primero fue diagnosticado de Adenoma Tubulovelloso y el otro caso fue un Adenocarcinoma Tubular, ambos de la Ampolla de Váter, en ningún caso se requirió de conversión a técnica abierta, no hubo mortalidad postoperatoria, los tiempos operatorios fueron de 420 y 360 minutos respectivamente. En los dos pacientes tuvimos fístulas que fueron manejadas conservadoramente, el tiempo de hospitalización medio fue de 25 días. Actualmente los pacientes no presentan evidencia de actividad tumoral y el índice de satisfacción por parte de los pacientes fue calificado como excelente. In the last decade the advances and the usefulness of laparoscopy have been very good for surgeons, however pancreaticoduodenectomy or Whipple procedure, was considered as a procedure beyond the means of laparoscopy by most surgeons. This descriptive study is carried from the month of August to November 2010, to perform surgery on two laparoscopic patients, the first was diagnosed as tubulovillous adenoma of Vate ampulla anthe other case was a tubular adenocarcinoma of Vater ampulla, none of these cases requiered conversion to open surgery time was 420 and 360 minutes respectively, The two patients had fistulas that were managed conservatively, the average of hospital stay was 25 days. Currently patients have no evidence of tumor activity and the level of satisfaction by patients was rated as excellent. El angiosarcoma hepático conocido como sarcoma de Kupffer, es una neoplasia maligna infrecuente, con una incidencia del 0.2 al 1% de todos los tumores hepáticos. Se caracteriza por un crecimiento rápido, con una infiltración extensa de las células anaplásicas que se derivan de los vasos sanguíneos. Metastatiza tempranamente a pulmón y bazo. Presenta una edad pico entre los 60 y 70 años, con una relación hombre mujer 4:1. Se ha relacionado con la exposición al cloruro y vinilo y con el Thorotrast, productos químicos usados en la fabricación de plásticos y medios de contraste intravenosos. La sobrevida usualmente es menor de 6 semes después del diagnóstico, debido a su alta tasa de recidiva. Presentamos el caso de una paciente femenina de 76 años considerando que el diagnóstico es complejo por la clínica y las pruebas de imágenes, las cuales suelen ser inespecíficas. The hepatic angiosarcoma also know as Kupffer sarcoma is an uncommon neoplasm, with and incidence of the 0.2 to 2% of all the hepatictumors. It ie characterized by an accelerated growth, with and extensive infiltration of the anaplasic cells that are derived from blood vessels. It metastesizes to lung and spleen. It presents an age pick between the 60 and 70 years, with a male female ratio of four to one. The risk has been realated with the exposure to chloride of vinyl and with the Thorotast, Both chenical products used in the production of plastics and in medical intravenous examination respectively. The patient usually has an overlife of 6 months or less after the diagnosis. We present a case of a 78 year - old female patient. El melanoma maligno es una neoplasia de melanocitos o de las células que se desarrollan de éstos. Es una rara neoplasia con un agresivo comportamiento que le confiere un mal pronóstico. Se presenta el caso de una paciente de 50 años que consulta por la presencia de una tumoración vaginal asociada a sangrado y a una adenopatía de 2 - 3 cm en la región inguinal derecha. La citología por aspiración con aguja fina del ganglio fue positiva para melanoma. La paciente es sometida a tratamiento quirúrgico. La histología informa un ganglio positivo y un melanoma de 5-6 cm de la pared vaginal, ulcerado. Se administra interferón alfa 2 recombinante. Tras un seguimiento de 32 meses, la paciente permanece libre de enfermedad. Aunque la cirugía es el tratamiento electivo, la inmunoterapia adyuvante debe considerarse en aquellos pacientes con enfermedad no localizada. Malignant melanoma is a neoplasm of melanocytes or of the cells that develop from melanocytes. Primary malignant melanoma of the vagina is a very rare neoplasm with aggressive biological behavior and bad prognosis. A case of a 50 year female patient with the diagnosis of malignant melanoma of the vagina is presented. A vaginal tumor with bleeding
and inguinal lymph node are found on physical exam. The fine- needle aspiration cytology of the lymph node is positive for diagnosis of melanoma. A wide surgical excision of the lesion with a margin ofnormal skin and therapeutic lymphadenectomy are performed. The results of histological examination identify a malignant melanoma with ulceration of 10 mm of depth and one metastatic node.
Interferon a-2b is administrated. the patient is free of symptoms afetr 32 months of follow - up
Although, surgical management is treatment of choice, adjuvant therapy that complements surgery is needed to prolong disease-free survival and improve the cure rate.

Los hamartomas hepáticos son tumoraciones raras, apenas 200 casos registrados a nivel mundial. Son más comunes en varones de 18 a 24 meses de edad, infrecuentes en adultos. Surgen del mesénquima de la tríada portal. Es característico el crecimiento abdominal lento y progresivo. Por su carácter benigno es importante diferenciarlas radiológicamente de otras masas hepáticas. Los exámenes de laboratorio son normales excepto por la alfafetoproteína, que puede elevarse y confundir el diagnóstico. Si el tumor alcanza gran tamaño produce efecto de masa, dificultad respiratoria y edema de extremidades inferiores. El tratamiento es quirúrgico y su pronóstico es favorable. Presentamos este caso debido a que es una patología poco frecuente, que a menudo puede pasar desapercibida y debe ser considerada dentro del diagnóstico diferencial de masas abdominales. Mesenchymal hamartoma of the liver (MHL) is an oncological benign entity. Fewer than 200 cases have been reported. It usually occurs in children under 2 years of age and slightly more common in boys, although older patients have been described. It arises from portal triad mesenchyme. Patients typically present with an asymptomatic abdominal mass. Radiological differentiation from other hepatic masses is important because of its benignity and potential for surgical cure. Sometimes alpha-fetoprotein (AFP) is elevated, confusing the diagnosis. When tumor is bulky, mass effect my cause distress and lower extremities edema.Surgery is the treatment of choice with a good prognosis. MHL is an extremely rare benign livertumor that must be considered as an abdominal mass differential diagnosis.
