A pesar de existir un avance tecnológico cada vez más sofisticado en la detección del cáncer, esta enfermedad continúa siendo un problema de salud pública en todo el mundo. Desde el primer trabajo publicado por George Von Revesy, en 1943, los estudios biológicos y de investigación clínica con radioisótopos se han sucedido como testimonio de su aplicación en distintas ramas de la Medicina. De inicio las pruebas ofrecían resultados cuyos datos permitían solamente un criterio de funcionalidad, lo cual aún siendo útil, analizándolo con rigor científico y a través de una exigencia no acorde con la experiencia, concedía unos limites muy estrechos a su campo de aplicación. El interés suscitado por estas técnicas originó su propia evolución hasta conseguir datos compatibles con la morfología del órgano o sector estudiado.

Despite the existence of a technological progress increasingly sophisticated in the detection of cancer, the disease continues to be a public health problem throughout the world. Since the first paper published by George Von Revesy, in 1943, the biological studies and clinical research with radioisotopes have been happened as a witness to their application in different branches of medicine. To start the tests offered results whose data allowed only one criterion of functionality, which still remain useful, discussing with scientific rigor and through a requirement, not in line with the experience, he gave some very narrow limits to its field of application. The interest aroused by these techniques originated their own evolution to achieve data compatible with the morphology of the organ or sector studied. |

La tomografía por emisión de positrones (PET) con tomógrafo multicorte al presente tiene un gran impacto en la práctica médica, y se ha constituido en una auténtica revolución dentro del diagnóstico por imagen con aplicación especial en el paciente oncológico. La imagen PET-CT con FDG es hoy en día el mejor ejemplo de cómo es posible mostrar las formas de incorporación de la glucosa a los diferentes grupos celulares del organismo. La imagen PET-CT ha permitido hacer realidad algo largamente anhelado dentro del mundo del diagnóstico: Fusionar dos especialidades complementarias como son la Medicina Nuclear (PET) y la Radiología (CT), concretando este momento de transición imagenológica que significa la sinergia de las imágenes morfo-metabólicas. Este artículo presenta una revisión breve de los principios básicos, lo elemental, la rutina en el manejo de la metodología PET/CT, ciclotrón, la generación de un radiofármaco, la adquisición de la imagen, y principales indicaciones oncológicas. Positron emission tomography (PET) with multislice CT scan has a major impact on medical practice and has become a revolution in diagnostic imaging with special application in cancer patients. The PET - CT imagiung with FDG is now the best example of how it is possible to show the incorporation of glucose to different cell groups of the body . PET-CT image has long yearned to accomplish something in the world of diagnosis: Combining two complementary specialities such as Nuclear Medicine (PET) and Radiology (CT), specifying this time transition imaging means images synergy morpho .- metabolic. This article presents a brief review of tha basics, routine in the management of the methodology PET / CT , cyclotron, the generation of a radiopharmaceutical, the image acquisition, and major oncology indications. Introducción.- Linfoma de Hodgkin (LH), representa el 14% de las malignidades linfoideas. La distribución de los subtipos histológicos varía de un país a otro; en Estados Unidos la esclerosis nodular es predominante y la celularidad mixta es más frecuente en América Central y Sudamérica. La tasa de cura excede el 80% con tratamiento, sin embargo en estadío avanzado, factores pronósticos pueden ayudar a identificar pacientes en riesgo de recaída que pueden ser incorporados a tratamientos intensificados. Hodkin´s Lymphoma (LH) represents 14 % of the lymphoid malignancies. The distribution of histological subtypes varies from one country to another. In USA, nodular sclerosis is predominant 0and mixed cellularity is more frequent in Central America and Soth America. The cure rate exceeds 80% with tratment, nevertheless in advances stage prognostic factors may help to identy patients at risk of relapse that can be incorporated into intensified tratments. Material and Methods.- A 10 years - review was realized for LH of the adult at Oncological National Institute - SOLCA from Guayaquil , between January 1998 and December 2007. Results.- 142 cases were found. Nodular sclerosis was the most frequent subtype (51.4 ) 45,8% of patients presented B sympyoms, 4.9 % with extranodal location, 8.5 % with bone marrow infiltration. The rates of Complete Response (RC) to ABVD in our Institute come to 62.7% however when the chemotherapy (QT) was administered with adequate intensity, the RC rates exceeded 90%. It was found that 71.8 % of patients were missing either treatment or follow - up because we do not have an adequate program for that. Conclusions.- It is urgent to created conscience that the delay in the help at the appropiated moment means to lose the opportunity to cure patients, leading to greater expenditure of resources. Introducción.- Los Sarcomas de Partes Blandas, son considerados un grupo de tumores poco frecuentes, pues su incidencia supone menos del 1% de todas las neoplasias, agrupadas en diferentes subtipos, ambos criterios (baja incidencia y diversidad de tipos) ayudan a explicar las dificultades para investigar y desarrollar con rapidez modelos terapéuticos eficaces. Soft tissue sarcomas appear at any age, although more frequent after the fourth decade of life. There is a realationship between some histological types of presentation. The term Soft Tissue sarcoma refers to a variety of rare tumors that originate from structures that connect, support or surround other organs and tissues commonly called soft tissue or part, so they area grouped as heterogeneous tumors tha show common histological features that are due to its histogenesis . In this paper we describe the clinical characteristics such as age of onset, sex, histological type, stage of disease and anatomical location. Material and Methods.- 53 patients with soft tissue sarcomas of head and neck from the National Institute of Oncology and Radiobiology were analyzed, between 1975 and 2005. Results.- The age of highest incidence was childhood with great foundness for the orbital region, within the hitological sbtype, the most frequent was Rhabdomyosarcoma. The most used tratment was surgery followed by radiotherapy, with poor response to treatment for a complete remission of 18.9% and 81.9% partial, where the most common clinical stage at diagnosis was III for 44.7%, followed by stage IV for a minimun 30.2 % with predominance in males compared to females. Conclusions.- Soft tissue sarcomas of the head and neck occur in all age groups, located in any region of the head and neck, but more often in orbit, mouth middle ear and nasopharynx.  La incompatibilidad sanguÃnea en el sistema ABO entre el donante y el receptor es un hecho de observación frecuente en el trasplante alogénico de células progenitoras hematopoyéticas debido a la presencia de anticuerpos en el receptor frente a hematÃes del donante. Aunque estos pacientes tienen mayor riesgo de complicaciones inmunohematológicas, la incompatibilidad de grupo sanguÃneo, no afecta a la incidencia de Enfermedad Injerto Contra Huésped o la supervivencia del trasplante. Lo relevante para el hematólogo es que esta diferencia puede ser anticipada y reducida a su mÃnima expresión con polÃticas transfusionales especÃficas. Se describe un paciente portador de Leucemia Mieloide Crónica sometido a Trasplante Alogénico con Donante emparentado e Incompatibilidad ABO.  Blood incompatibility in the ABO system between donor and receptor is a common event observed in allogenic hematopoietic progenitir cells due to  the presence of antobodies  in the receptor against the donor red cells. Although these patients have an increased risk of inmunological complications, blood group incompatibility does not affect the incidence of graft versus host disease or graft survival . What matters for the hematologist is that this difference can be anticipated and reduced to a minimum with specific tranfusion policies. We report the case of a patient with chronic myeloid leukemia receiving allogeneic  related donors and ABO incompatibility. El leiomioma es el tumor intramural extramucoso más frecuente del esófago, localizado en la mayoría de los casos en el tercio distal. Su incidencia es variable, de un 0.005 a 5.1%. Es más frecuente en varones, en una proporción de 2:1 y con un pico de incidencia en la tercera a quinta década de la vida. En el 10% de los casos son múltiples. Leiomyoma is the most frequent extramuccosal intramural tumor of the esophagus, most of the cases is located in the third distal. Its incidence is variable and ranges from 0.005 to 5.1 % . It is more common in males by a ratio of 2:1 and a peak incidence in the third to fifth decade of life. In !0% of the cases they are multiple . Their sizes widely, from less than one cm in diameter to 29 cm . From 10 cm on they are considered giants . The treatment is surgical removal, either by minimally invasive techniques, endoscopic or thoracolaparatomy according to tumor size. We report a case of a young patient with a giant leiomyoma which was surgically resolved with resection of the tumor mass that involved the 2/3 of the lower esophagus and 1/3 of the proximal stomach by a thoracolaparotomy and esophagus gastric anastomosis. Los linfomas afectan a más de un millón de personas en todo el mundo. Anualmente la incidencia aumenta en un 3%. El linfoma primario difuso del mediastino de células grandes tipo B es una entidad clínico-patológica que se produce en el timo y que afecta principalmente a pacientes adultos jóvenes. Se presenta el caso de un paciente masculino de 29 años con un linfoma primario mediastinal de células grandes, el cual se presentó como una masa mediastinal con síndrome de la vena cava superior, invasión de estructuras vecinas intratorácicas y derrame pericárdico. En el estudio histopatológico se observó un infiltrado difuso de células B grandes hendidas y no hendidas, con núcleo multilobulado y esclerosis hialina con efecto de compartimentalización. Algunos pacientes responden bien a la terapéutica con quimioterapia agresiva mientras que otros evolucionan a enfermedad progresiva, como ocurrió en nuestro paciente. About a million people are estimated to be affected by a lymphoma. the incidence rate increases about 3% annually. Primmary mediatinal B - cell lymphoma that arises in the thy mus and mainly affects young adults. We reported a case of a 23 years old man with prmary mediastinal diffuse large B -cell lymphoma. He presented a rapidly proliferating tumor in the anterior mediastinum with superior vena cava syndrome, invasion of intrathoracic structures and pericardial effusion. The histopathologic study demonstrated a diffused infiltration with a population of large cells (B cells) with multilobated nucleus. Sclerosis and fine comparmentalized fibrosis were observed in the tumor. This is considered an aggresive lymphoma which may be cured with aggressive chemotherapic. Some patients have a progressive disease with a bad prognosis, as tha case reported. Key words.- Mediastinal lymphoma, histopathology, chemotherapy. |

El carcinoma de Merkel y el Tumor Neuroectodérmico Primitivo corresponden a tumores de células pequeñas y azules. Su diagnóstico diferencial es difícil dada la similar morfología y la superposición del panel de inmunohistoquímica, siendo necesario el uso de biología molecular. Se reporta el caso de un hombre de 54 años, con tumoración en muslo, realizándose biopsia, cuyo resultado histopatológico evidencia Tumor Neuroendócrino Maligno, Synaptofisina y Enolasa Neuronal Específica positivo, sin expresión de CD99 ni de CK20. Se evalúa la presencia de la traslocación (11:22), cuyo resultado fue negativo. Se destaca la importancia del diagnóstico diferencial entre ambas entidades dado que presentan tratamientos y pronósticos diferentes. La morfología del tumor, tipo células redondas y azules, y la inmunohistoquímica sólo indican la presencia de un tumor neuroendócrino maligno. La confirmación surge a partir de la presencia de la traslocación 11;22 en el tumor neuroectodérmico primitivo. Palabras Claves: Tumor, biología molecular, inmunoshistoquímica. |

Merkel cell carcinoma and primitive neuroectodermal tumor are small round blue cellt umors. Their diffrencial diagnosis is difficult due to similar morphology and overlapping of immunohistochemical panel, requiering the use of molecular biology. We report the case of a 54 year - old man, with a mass in the thigh whose biopsy revealed a malignant neuroendocrine tumor, synaptophysin and neuron - specific enolase positive, and CD99 and CK20 negative. We evaluated the presence of the translocatrion (11:22), which was negative. The diffrential diagnosis between these entities is important because they present tratments and prognoses. The morphology of the tumor, round and blue cell type, and the immunohistochemistry panel only indicate the presence of a malgnant neuroendocrine tumor. The confirmation comes from the presence of the translocation 11:22 in primitive neuroectodermal tumor. Key words.- Tumor, molecular biology, inmunohistochemestry. |

At the present time all Health Organizations have increasing health care demands, a fact is related to some factors which area common in Oncologic Neurosurgery. They can be : 1) Knowledge of subjects related to health , some of them specialized , due to better source of infomation accesible with technology 2) Increase life hope of the patients. 3) Medical technology offers better and more efficient tratment 4) Diagnosis procedures allow better resolution and detection of lesion 5) Patients have better access to places to receive tratments. To face this increase in the demand of patients there are limited resources, hece is necessary to optimized action to get efficiency in the results. In all these processes act the Health Economics.
